Enrolment Declaration *,taken from the Chamber,will be filled and signed by the authorities.at the 11^th item,the actually job will be written ,and your Job Group will be definite after the acception of Board Committee.

Petition*

Copy of notary approved main agreement ( 2 copies )

Copy of, notary approved, translated certificate, taken form registration management ,or competent authority which the company enrolled.

3 copies of Decision, taken by the authorities of the company during the starting of the branch ( notary approved )

3 copies of, notary approved, translated Power of Attorney Certificate of permission of the Ministry (2 copies)

Declaration of Signature, arranged by the representative of the branch trade name ( 2 copies )

Copies of id-card or passport of the branch representative and residence paper ( 2 copies )

Written contract, arranged according to the 29^th item of Trade Registration Regulation

Cost of Chamber enrollment will be taken in cash because it will be enrolled with Trade Registration at the same time, so in that year, annual membership fee will not be taken

If the company is working in a sector which takes part in our city the copy of MASTER CERTIFICATE of one of the partners or of a master working for them, and also a signed petition by the authority of the company showing that master works for them.

The jobs in scope in our city are given in the attached list.

30^th item of the 3308 no Law, changed with 4702 no Law says: ‘Vocation Chambers must get a MASTER CERTIFICATE from the members who will enroll as the owner of the company.’ If you can’t present the Master Certificate, a written contract about you company opening licence or Master Certificate will be filled and signed.
